Sliding Fee Scale Program
As a Federally Qualified Health Center, we are able to offer a discount on some services based on a patient’s household income and family size

The Sliding Fee Discount Program for health care services is available to patients. The scale is updated annually and is based on the most recently published Federal Poverty Guidelines. Eligibility will be determined based upon the income of all persons residing in the household.

Please stop by your nearest ARMS, Inc. location to fill-out a sliding fee application and bring your eligibility documentation requirements.

Eligibility documentation could include:

Last thirty (30) days proof of income;

Previous year Federal Income Tax return

Bank statements
-
Court documents: citing child support or alimony awarded by the court.

Most recent award letter from Social Security or Veterans Administration, local Social Services indicating benefit amount.
Verification of Sliding Fee Scale is required every six months.

Your current sliding fee or insurance card must be presented at each visit.
